90 Day Initial Commitment: Membership requires a minimum commitment of ninety days from the date of purchase.

Month to Month Renewal: After the initial commitment, membership automatically renews month-to-month until canceled.

To Cancel: Email info@sumitsyogacolumbia.com at least thirty (30) days before your next billing date.

Sumits Hot Yoga Columbia LLC reserves the right to adjust membership pricing with at least thirty (30) days written notice. Existing promotional or grandfathered pricing remains at the studio’s discrestion.

Payment Authorization: I authorize Sumits Hot Yoga Columbia LLC to charge my payment method on file for the membership fees on the billing date shown on this form.

Payment Failure: If a payment is declined, I agree to update my payment information promptly. Returned payment fees and a late fee may be asessed if permitted by law. Membership privliages may be suspended until account is brought current.

Non-Transferable/Non-Refundable: Memberships and mat storage are non-transferable and non-refundable.

The studio reserved the right to refuse service or terminate membership for repeated policy violations, abusive behavior, or non-payment.
